Upon arrival, you will be greeted by our front desk team. They will gather the necessary information for your initial visit. Your therapist will perform a thorough evaluation and begin your initial therapy session.
The initial evaluation will last a total of one hour to 1 hour and 15 minutes.
Follow up appointments will last anywhere from 45 minutes to one hour.
To optimize your evaluation and time with your therapist, we kindly request you arrive 15 minutes prior to your appointment in order to fill out/review necessary documents.
Our office is located on 24-08 32nd Street Suite #1002D, Astoria, NY 11102. Our entrance in on 32nd street on the ground floor. There are two steps once you enter the building. Please contact us if you have difficulties with stairs. We have another entrance that does not require stairs.
Before your first appointment, we will contact your insurance company and let you know what your responsibility will be for each visit. We also offer self pay rates if you do not wish to use insurance.
We accept most insurances. We will be able to call your insurance company and let you know how much you will be responsible for each visit prior to being seen.
Please be prepared to bring the following on your first day:
Driver's license/photo ID
Prescription (if applicable)
Insurance card
Athletic clothing (we also have clean gym clothing if you would like to borrow for session)
Diagnostics (MRI, x-ray reports)
We value our patients and believe that you deserve the best possible care. Our treatments are highly individualized in order to maximize your recovery. We prioritize your time and ensure that we are providing up to date treatment in the field of OT/PT based on most recent practice.
Under NYS law, you can see a physical therapist or occupational therapists for 10 visits or over a span of 30 days, whichever comes first. You will need a prescription for therapy if are treated beyond this time frame. Your therapist will be able to guide you on the appropriate doctor you should see for your specific condition. Visit frequency and duration will be determined in collaboration with the patient, therapist and referring physician.
COVID-19 response:
Our number one priority is the health and safety of our patients and staff. At SOMA we take COVID-19 seriously and ensure that we will provide a safe and clean environment for patients and staff. All persons in the clinic are required to wear a mask throughout their time in the clinic. Masks will be provided if needed.

Typically, you will be seen by one therapist throughout the course of your care. There are instances (i.e. if your therapist is on vacation) that you will be seen by a diffrent therapist. If you are treated by a different therapist, we will ensure that the treating therapist will collaborate with your primary therapist to ensure your plan of care is followed and progressed as indicated.
Expect to spend anywhere from 1 hour to 1 hour and 15 minutes for your initial evaluation.
All follow up appointments will last approximately 45 minutes to one hour.

You are allowed to see any therapist or go to any office of your choosing. We will collaborate with your physician's office as indicated in order to maximize care.
